Please complete this application only if you have received confirmation of participation for your child for the upcoming school semester. You don't need confirmation if your child is a returning student from the previous semester. Please note that an application can only be submitted after the new fee has been determined in the Swedish School Annual Meeting, usually held in September or October. Note that payment through PayPal must accompany the application to be considered.
Effective immediately and until further notice, I acknowledge that I will not seek to have the Swedish School Association in Seattle held liable in the event that any accident, injury, loss of property, or any other circumstance or incident occurs during, or as a result of, my child's participation in any school activity, either on school premises or during a field trip. This release of liability includes accident, injury, loss, or damages to the student, as well as to other individuals or property which may result from the student's participation in any event organized by the Swedish School Association in Seattle. I hereby release and agree to hold harmless the Swedish School Association in Seattle, its officials, agents, and employees, from any claims arising out of my child's participation in any school activity, either on school premises or during a field trip.
